Minors must have a parent or legal guardian fill out and submit this form and accompany them on the trip. The name of the parent or legal guardian filling out this form and accompanying the minor should be added in the space provided below for special instructions.

Rooming arrangement

I would like to be matched with a room mate. I understand that Bible Expeditions will do their best to match me with a same-sex person in a double room, but if they are unable to find a match for me, I agree to pay the additional supplemental cost for a single room.
